What information will we disclose to your provider?

We may at their request provide anonymous consolidated statistical data including the following:

  • Gender
  • Age range
  • Medical questionnaire’s
  • Test recommendations

NOTE: We will only disclose this information within anonymous groups of information.

We may disclose whether or not you have used the system. This will be or the sole reason of improving uptake of the system.

What information will we not disclose to your provider?

  • Any information that will identify you personally, other than usage of the system
  • Test input values

When will we contact you?

  • We will notify you when a new test is due by either e-mail or Text
  • We may contact you by e-mail of text in order to carry out surveys designed to improve our service or your usage of the system
  • In order to provide you with support
  • In order to improve your usage of the system

Will we use your information for anything else?

  • We may disclose consolidated statistical data to research institutions for research purposes only. This data will always be anonymous
  • We may also use consolidated statistical data for our own research purposes, to analyse trends and improve the Key to LifeTM
  • If we want to use your information for any other purpose than those listed above, we’ll contact you and ask you whether you agree – if you don’t or we don’t hear back from you, we won’t use it for any other purpose

What happens when I leave the Key to Life system?

When you leave the system your name, telephone number, email address, and date of birth, (other than the year of birth), will be removed from our system. Your test data will remain on the system as anonymous data.

Changes

Upon receiving any notice of change to the Privacy Policy you will be required to re-confirm your consent to the Privacy Policy before continuing to use the service.
